By bringing together skills, knowledge and experience, we’re all able to provide services that work together and are consistent for the people who we support.

This way, we can work with people when they need us, not just when they reach crisis point.

Through this work, we aim to create healthier, happier and stronger communities.

How do we work with people?

We provide proactive and early support for anyone who is vulnerable due to their health, well-being or situation and wants to make change happen in their lives.

It could be as simple as getting advice, being given contact details, or people may need support that requires a bit more time, such as working out a plan of how they want to overcome difficult or overwhelming things in their life.

Some of the main areas people choose to focus on:

  • Physical, mental and financial well-being
  • Connections to communities
  • Housing situations

 

We have Keyworkers who coach individuals to develop their own strengths, relationships and abilities as they work towards the kind of life that they want. This will usually start with a conversation, where we listen, rather than carry out an assessment or bombard people with questions.

We don’t believe in categorising people and will start with their strengths, empowering them to find their own solutions.

Based on this, we create an action plan together. Ultimately, it’s all about believing in the person.

My TPA Keyworker has supported me over the last year, she has helped me overcome so many problems, without her support I don’t know how I could have got through it. For every obstacle there was a solution and she pointed me in the right direction. She always followed up and made sure I was ok and reaching my full potential. I genuinely felt that I could openly talk to her, she has given me more strength and courage to believe in myself – I will always be grateful for her support.
